Instructions
- Performance evaluations must be completed for each candidate for each practicum or internship
- Evaluations must be completed by both DePaul Supervisors and Site Supervisors (and candidate as required by program)
- Turn all evaluation forms in to the Coordinator of Advanced Programs.
